How to make a claim
It is important to notify Proclaim or Uniting Resources Insurance Department of any claim or potential claim as soon as practicable and appropriate action should be taken to minimise the damage and /or loss.
Do not admit liability, even if you think you are at fault. This may prejudice the insurer’s chances of recovery and may result in a reduced settlement.
Incident / Claim Forms can be downloaded from either Proclaim website or from the Uniting Resources website on the Intranet.
It is important to understand what needs to be reported; the following outlined the minimum criteria.
1. Incident – An incident is any event or occurrence where any of the following apply;
a. There is property damage.
b. Property is considered missing or lost.
c. There is an injury to any person while on property belonging to the church.
d. There is an injury to any person while on property in the legal control of the church.
e. There is an injury to any person while they are considered under the care of the church.
f. Staff employment is terminated and circumstances may involve any of the above.
g. There has been actual breech of any laws or statutes.
h. Entities receive subpoenas or legal requests.
i. Not withstanding any of the above, an incident also includes near misses where any of the above may have occurred or that an ordinary person considers the result of an action or situation may result in any of the above occurring.
2. Property – Physical items, includes Buildings, Contents, Cash, Personal Belongings that the church is legally responsible for etc.
3. Injury – Harm, includes actual physical harm and trauma.
Claims may be lodged electronically or by mail.
